A 38-year-old man presents to a clinic with a sore throat and dysphagia. He recently had receptive oral sex with two different men. On physical examination, the posterior oropharynx is erythematous without an obvious exudate. A pharyngeal swab, first-catch urine, and rectal swabs are sent for nucleic acid amplification testing (NAAT) for Chlamydia trachomatis and Neisseria gonorrhoeae. The pharyngeal swab is positive for N. gonorrhoeae, but all other tests are negative. He weighs 83 kg and has no allergies to antibiotics.
